Risk analysis in plant commissioning: the Multilevel Hazop
نویسندگان
چکیده
The paper deals with risk analysis and management in the realisation of process plants by engineering and contracting companies. Specifically, a new analytical methodology is developed to manage the typical risks of plant commissioning. Following a detailed study of the commissioning process and its criticalities, Hazop is selected as the most suitable approach in forecasting particular risks of this project stage. Starting from the limitations of both Hazop and Human Hazop (the extension of the technique to the field of procedures performed by humans), a new technique (Multilevel Hazop ) is suggested which identifies in advance all the criticalities of each commissioning activity and defines actions to reduce risk and optimise dedicated resources. After implementation of Multilevel Hazop in a real commissioning process, a new version of the technique (Two Stage Multilevel Hazop ) is proposed. This allows a significant reduction in implementation costs, justifying use of the technique both in innovative or critical cases, where the expected saving from risk reduction is high, and in general contexts. q 2002 Elsevier Science Ltd. Hazard Identification of the Offshore Three-Phase Separation Process Based on Multilevel Flow Modeling and HAZOP
HAZOP studies are widely accepted in chemical and petroleum industries as the method for conducting process hazard analysis related to design, maintenance and operation of the systems. Different tools have been developed to automate HAZOP studies. In this paper, a HAZOP reasoning method based on function-oriented modeling, Multilevel Flow Modeling (MFM), is extended with function roles. A graph...

HAZOP studies are widely accepted in chemical and petroleum industries as the method for conducting process hazard analysis related to design, maintenance and operation of the systems. In this paper, a HAZOP reasoning method based on function-oriented modelling, multilevel flow modelling (MFM) is extended with function roles to complete HAZOP studies in principle. A graphical MFM editor, which ...
متن کاملA Functional approach to HAZOP studies
This paper presents a functional approach to HAZOP studies in which the plant is divided along strictly functional lines starting from the purpose of the plant and continuing down to the function of a single node, e.g. a pipe section. The approach leads to nodes with simple functions such as liquid transport, gas transport, liquid storage, gas-liquid contacting etc. From the functions of the no...
متن کاملRisk Assessment System for Verifying the Safeguards Based on the HAZOP Analysis
In recent years, serious accidents in chemical plants frequently occurred in Japan. In order to prevent accidents and to mitigate process risks, to re-evaluate risks which consider the reliability of existed safeguards in chemical plants is needed. The chemical plant is obligated to provide and maintain a safe environment for people that live in such circumstances. Plant safety is provided thro...

Background; Prevention of accidents—a crucial requirement in oil industries—involves hazard recognition, risk assessment, and corrective actions. The purpose of this study was to compare the ETBA and HAZOP techniques for risk assessment in a gasoline refinery unit. Methods; In this case study, data were collected using process flow diagram, walking–talking through method, piping and instrument...
